We wish to inform our patients about a significant change to our appointment system that will improve the way our surgery delivers care to our patients. GP practices across the Black Country and nationally are moving to a 'modern general practice' model to make access to care and treatment easier for patients and to help improive patient satisfaction.

From January 2026, we are starting our new system where when you have concerns you will need to submit a form and you will be signposted or offered an appointment with the appropriate clinician within a reasonable time frame. Please note that if you do not have access to the website and you want to book a routine appointment, please call reception and they will help you.

How does this work?

Initially, a reception member will establish if your symptoms and circumstances meet the criteria for other health providers, this could be the pharmacy. If you are fortunate enough to have symptoms that can be dealt with by a pharmacy, a referral will be made for you to go to a community pharmacy. You will then be contacted by the community pharmacy for them to complete their assessment, this can be done via telephone or an invite to attend the pharmacy within their opening hours.

It is vital that we have the correct contact information for you, this includes your telephone number and email address. If you know we have the incorrect information on your record, please let reception know as soon as possible.

Please note, for the GP to triage your request in a timely manner, it is important to provide detailed and accurate information about your symptoms. We can no longer accept 'personal' as a reason for an appointment. If the information is less detailed, it is harder for reception to triage your form and this may delay the time frame in which you receive an appointment.

During the weekends or bank holidays, please continue to use the 111 service or call 999 for life threatening emergencies. you may also be referred to 111/walk-in centre if we are at capacity for the day.
